Following feedback from you, we are pleased to announce that we now have the registration documents available for you to download in Microsoft word format directly from this site.
To do this, click below on either the icon and choose ‘save’. Once the file has downloaded you will need to print out and complete one copy for every member of your family over the age of five who wishes to register with us. The completed form should then be returned to either the Green or Red Practice at either Waterside or Blackfield Health Centres, depending on your preference of practice and location.
