We may need to contact you in an emergency so it is very important that we have your correct contact details on record.

 

If you change your address, telephone number, please collect a Changes in Registration Details Form from Reception, or, alternatively, you can download the Changes in Registration Details Form here. Once completed please return this form to Reception.

The results of blood tests and other diagnostic investigations are often available only after the surgery has closed. If immediate follow up is required the hospital or out of hours service may need to contact you urgently.

 

Please check the address and telephone number on the blood test request form are correct.
