Carers' Form

 

The Practice needs to update their records for patients who are carers so they may be given priority to receive a vaccination.  To do this, patients who are carers are asked to complete a form that may be downloaded from this site.  The form is in two parts and must be signed by both the carer and the patient.  You may access the form by clicking here to download and print a form that has to be completed by hand and returned to the Waterside (Hythe) Health Centre in person or by post.

Use of eConsult

 

Patients are requested not to either, ring the surgery or use eConsult to ask for a vaccination. If you ask using eConsult, it just creates extra work for the surgery to respond, to say that this is not possible. It is not possible to ask for a vaccination and you will be offered an invitation to receive your vaccination when it becomes available
