Each year we vaccinate in the region of 6000 people against flu. In past years this demand caused some congestion on our telephone system which meant that some of our patients had problems in contacting us to make their appointment. In response to your feedback we created this form. If you are having problems getting through on the telephone to book an appointment for your flu vaccine then you can complete this form and we will call you back to arrange an appointment within 72 hours. The form only needs to be completed once for each household.
