How to Update Your Contact Details

If any of your contact details change you can inform us by using one of the following methods:

 

  • Complete the Changes in Registration Details form. Click on this link to download the Changes in Registration Details Form and return it to us. This form is also available at Reception.

  • You could write us a letter

  • If you have registered for Patient Access you can change your contact details via the Patient Access Website. (If you would like to register for Patient Access click here for more information)

  • Or simply complete the form below which will be sent directly to our Registration team


IMPORTANT

 

If you  have changed your name, you must show proof of your name change, i.e. deed poll, updated passport, marriage certificate etc., before we can change your name on the NHS Database.
